is there one for spending 5000 Black Gems? can not remember if I've seen it or just the ones for spending 2500

No. Only 2500 for level 2 art and 7000 for level 3 art. They usually show up around the time of tournament.

There is also an offer to spend 15000 gems for a tier 4 artifact.

It shows up as an Admin message/offer in the inbox.

To recap:
There are several offers...
-spend 1000 gems for a tier 1 artifact.
-spend 2500 gems for a tier 2 artifact. (typically seen the most on this server)
-spend 7000 gems for a tier 3 artifact.
-spend 15000 gems for a tier 4 artifact.

But do the count as a failed quest if they come up and you do not buy them?

A very important question given the 150-200-50 quests we are given.

132/200 and 15k Gems right now would derail that for sure.

This quest if accomplished will be scored in quest with quest counter. However if failed it will have no effect on the mentioned quest counter. This quest can be accomplished only in scope of one-time special offer from Administration.

can a special offer be used twice, i.e spend 2500 get an artefact then spend another 2500 and get a second artefact or will it only work once per offer?

Once per offer -- technically a quest that is completed like the rest.

Update for tier 5 offer from admin:

-spend 1,000 gems for a tier 1 artifact.
-spend 2,500 gems for a tier 2 artifact.
-spend 7,000 gems for a tier 3 artifact.
-spend 15,000 gems for a tier 4 artifact.
-spend 30,000 gems for a tier 5 artifact.

Saw this on the UK server and wanted to post it here just for reference as I'd never seen this kind before and maybe there will be similar ones in the future.
